'Provider name was not recognized by the server' error

Question:

What is the cause and how do I fix this error?

Answer:

The problem can be caused by making changes to the server, but 
not rebuilding the entire project, so the EXE and TLB were out 
of sync. Simply rebuild the server with 'Build All'.
